The Opportunity Maintains efficient and effective operations of the Centralized Information (referral?) Services (CIS) to provide patient care needs in collaboration with the manager; - Acts as a liaison between the manager, physicians and other members of the CIS team to facilitate effective communication and collaboration; - Assumes leadership roles within the Children's Health Program including the Centralized Information Services Team and Clinic Meetings, Quality and Patient Safety Committees (including review of all SIMS), Accreditation Teams, etc. The Clinical Leader may delegate shared responsibility for these committees where appropriate; - Participates in, and develops, service planning for Centralize Informationservices to predict future needs and new directives and statistical data concerning patient/team activities; - Provides leadership in care team discussions and provides input and energy into efforts at improving clinic efficiency and functioning; - Uses the Nursing Process to full scope for communicated with stakeholders, team members and community physician offices; - Accurately assesses patient/family needs during clinic visits and telephone consultations; independently develops an individual nursing plan of care based on patient/family nee
