Job Overview An excellent opportunity has arisen for a Band 6 Community Occupational Therapist to join the South Staffordshire Integrated Community Occupational Therapy Team : Adult Social Care. Based in Lichfield/Burton, this temporary position is ideal for an experienced Occupational Therapist with a passion for supporting adults with physical disabilities and long:term health conditions. The role focuses on enabling individuals to maintain independence within their homes and community through targeted occupational therapy interventions. Key Responsibilities * Provide assessments and interventions to support adults with physical disabilities and long:term health conditions. * Develop personalized care plans to promote independence and quality of life. * Work closely with individuals and their families to identify needs and deliver appropriate solutions. * Collaborate with a multidisciplinary team to ensure a coordinated approach to care. * Monitor and review progress, making necessary adjustments to interventions. * Contribute to the teams goals by engaging in service development and clinical audits.
