Recovery Coordinator Location: Whinney Hill, DH1 3BQ Salary: A GBP 24,020 : A GBP 30,790 per annum An exciting opportunity for a Recovery Coordinator has arisen within our recovery co:ordination team based at Centre for Change in the City Centre of Durham. The role of a Recovery Coordinator is to manage a service user caseload, devising, implementing, and reviewing ongoing recovery plans and treatment, liaising closely with the full range of workers and skills within the service team, including family members and significant others as appropriate. You will help set goals in recovery and help the service user achieve their goals in a safe and supportive way. The Recovery Co:ordinator role is a diverse and rewarding role, Key Duties: * Manage a caseload, devising, implementing, and reviewing ongoing recovery and treatment plans. * Facilitate and promote individuals progression from initial engagement, through structured treatment, to self:sustained recovery. * Reduce substance:related harm to individuals and the wider community. * Working collaboratively with key partners and colleagues to achieve the best health and well:being outcomes for individuals. * Work within the community in GP surgeries, Mental health services and other partner agencies If you would like to work with a team that really makes a difference : we would love to hear from you. Working within central Durham and the wider county, you will be required to work as part of a team to provide recovery planning and case management to substance misuse clients (Drug and Alcohol) at different stages of their recovery journey, from the assessment and engagement phase providing holistic and structured intervention and psychosocial interventions.
