Job Description

We have partnered with a cutting-edge healthcare partnership and training practice in South-East London. They have a list size of approx. 21,000 patients and are dedicated to improving patient services and training and developing clinicians for the future. They are actively seeking a new Clinical Pharmacist to join their diverse and experienced team of healthcare professionals. Position Details: * Job Title: Clinical Pharmacist * Job Type: Full time/part-time hours available from Monday– Friday * Location: South-East London * Salary: £45,000 - £55,000 per annum, depending on experience, with an extensive benefits package including learning and development opportunities. * Additional Services: Complete range of medical services, including maternity care, baby clinics,immunisations, general medical and surgical care, minor surgery, contraception, screening, and health promotion for both men and women. * Support Team: Work alongside four GP Partners, Salaried GPs, Nurse Practitioners, Practice Nurses, Physician Associates, and Healthcare Assistants. About You:* GPhC registered pharmacist with experience working within a GP Practice setting. * Experience and an awareness of common acute and chronic conditions. * Effective communication of complex information via various channels. * Strong interpersonal, organisation, and IT skills, capable ofchallenging managerial and clinical practices. * May hold or be working towards an independent prescribing qualification is desirable but not essential as full training provided. Main Duties: * Work as part of a multi-disciplinary team to clinically assess and treat patients, utilising expert knowledge of medicines for specific disease areas. * Manage care for patients with chronic diseases, conducting clinical medication reviews to proactively handle complex polypharmacy cases. * Ensure accurate recording on clinical systems to meet QoF compliance, while contributing to meetingtargets and enhancing services. * Provide leadership on medicines optimization, quality improvement, and adherence to guidelines, while maintaining a clean and effective working area.
